For many women trying to conceive, learning they have a low egg reserve can be concerning. However, a diminished ovarian reserve (DOR) does not mean pregnancy is impossible. With the right knowledge, lifestyle adjustments, and available fertility treatments, many women with low egg reserve can still achieve their dream of having a baby.

What Is Low Egg Reserve, and How Does It Affect Fertility?

Low egg reserve, or diminished ovarian reserve (DOR), means a woman has fewer eggs in her ovaries than is typical for her age. This can make it more difficult to conceive, both naturally and with fertility treatments. However, it’s important to remember that DOR does not mean pregnancy is impossible. While it may take longer or require medical assistance, many women with DOR go on to have successful pregnancies.

Causes of Low Egg Reserve

Several factors can contribute to a diminished ovarian reserve, including:

  • Age: The most significant factor, as egg quantity and quality naturally decline over time.
  • Genetics: Conditions like Turner syndrome or Fragile X syndrome can impact ovarian function.
  • Medical Treatments: Surgeries on the ovaries, chemotherapy, and radiation therapy can reduce egg reserve.
  • Lifestyle Factors: Smoking, excessive alcohol consumption, and exposure to environmental toxins like pesticides and pollutants may negatively affect ovarian health.
  • Autoimmune Diseases: Some conditions cause the immune system to attack ovarian tissue.
  • Endometriosis: This condition can affect ovarian function and reduce the number of available eggs.

How Is Low Egg Reserve Diagnosed?

Doctors use a combination of blood tests and transvaginal ultrasound to assess ovarian reserve.

Can You Conceive Naturally with Low Egg Reserve?

Having a low egg reserve does not mean natural conception is impossible, but it can make it more challenging. While there are fewer eggs available, some may still be of good quality. Many women with DOR continue to ovulate and successfully conceive naturally. Other factors, such as sperm quality and overall reproductive health, also play a role.

Fertility Treatment Options for Low Egg Reserve

If you are not ready to conceive, you may consider egg freezing. Since your ovarian reserve is low, higher doses of stimulation medications may be required to retrieve an optimal number of eggs. If you have a partner, embryo freezing is another option.

For those ready to conceive, trying naturally for 3-6 months is recommended. If pregnancy does not occur, fertility treatments such as intrauterine insemination (IUI) or in vitro fertilization (IVF) may be beneficial.

In rare cases, a woman with a very low ovarian reserve may not be able to conceive with her own eggs. In such situations, egg donation may offer a viable alternative.

Success Rates of Fertility Treatments for Low Egg Reserve

The success of IVF or egg freezing depends on factors such as age, overall health, and egg quality. While women with a lower ovarian reserve may have fewer eggs retrieved, success may still be possible, especially with individualised treatment plans. Consulting a fertility specialist can help determine the best approach for your unique situation.

Coping with a Low Egg Reserve Diagnosis

Receiving a diagnosis of low egg reserve can be emotionally challenging. Here are some ways to cope:

  • Acknowledge Your Emotions: It’s normal to feel a range of emotions, from sadness to frustration. Allow yourself time to process these feelings.
  • Seek Support: Talk to your partner, friends, or a counsellor. Joining a support group can also be helpful.
  • Educate Yourself: Understanding your condition and available options can help you feel more in control.
  • Explore Treatment Options: Whether you choose to try naturally, undergo fertility treatment, or consider egg donation, there are paths to parenthood.

Having a low egg reserve does not mean you cannot have a baby. While it may require more time or medical assistance, many women with diminished ovarian reserve achieve successful pregnancies. By exploring fertility preservation options, seeking early intervention, and working with a fertility specialist, you can take proactive steps toward parenthood.
